Urgent question on how to fill out a deposit slip! please help if you know?

I am going to drop off my deposit slip in the drop box and I need help filling it out. I am putting cash in it so I don't know if I should sign my name where it says 'Sign here ONLY if cash recieved from deposit' . I asked my mom if I need to sign it and she said no because I'm not getting cash back but my gma said I need to sign it. Help me please! thank you =]
